We came across a rather alarming story in the New York Times last week that we wanted to call your attention to.

The story concerned a recent study published in the Journal of the American Medical Association. That study found that hospitals actually wind up profiting economically from surgical mistakes because insurance companies pay for longer stays and follow-up care that would not have been necessary had the surgery been performed properly in the first place.

The study authors said in order to change this, the economic model of health care in the U.S. needs to be altered to stop proving a de facto monetary incentive for providing poor care. A spokeswoman for a trade group representing insurance companies said from her group鈥檚 perspective, doctors and therefore hospitals are still making the most money by pursuing a quality-over-quantity approach; the more surgeries they perform, the more money they make, regardless of how well those surgeries turn out.

Now, we need to be clear about one thing 鈥 the authors explicitly wrote that there is no indication hospitals are deliberately making mistakes to earn more money. Rather, the issue seems to be that there is no financial penalty for providing substandard care (in fact, it鈥檚 the opposite).

Even so, the results of this study strike us as very problematic. We need to discourage patient harm in every way possible, and the fact that there is not enough of an economic 鈥渟ting,鈥 so to speak, when a surgical error occurs is a glaring problem that must be addressed post-haste.

For parents, there is little scarier than when their child is sick or injured. The need for instant care is crucial in many cases, and parents expect that doctors will give the young patients the best care that they can. However, the cases in which one child is given one level of care and a different patient is given a different level of care is not always due to medical need. Instead, a new study finds that the level of care correlates more closely with the preferences of the care provider.

The study looked at medical care of children in three New England states. While not a large geographical area, the level of care provided to children varied wildly. Some locales had twice as many emergency room visits for聽child injury聽and illness as did other locals; the rate of tonsil removal was twice as high in some areas than in others.

Researchers attributed some of the variance to the fact that pediatricians are not necessarily located in the areas that have a younger population and thus would need them more. However, others have suggested that the problem isn鈥檛 simply due to geography but rather a lack of consistent quality measures for pediatric care.

One fear that experts have about resolving the situation is that if every state comes up with its own list of quality measures for pediatric care to avoid聽medical malpractice, then clear guidelines across the country could become more muddled, not less. Fifty different guidelines wouldn鈥檛 necessarily lead to more clarity. However, if they do improve responsiveness and patient care, then it might still be progress.

Surgical errors happen more often than people might realize 鈥 and can be devastating for the patient. Preventable errors that take place in a New York operating room are often prime candidates for medical malpractice suits because of the devastation that they could cause.

Adding to some people鈥檚 outrage is the fact that many of these errors are preventable 鈥 and the way to prevent them costs a pittance. According to research, the most common item left behind inside a patient鈥檚 body after surgery is the sponge. These are relatively small pieces of material that can be hard to see after soaking up blood 鈥 so they may get left behind in surgery.

The simple way to prevent this, of course, is to keep an accurate count of how many sponges are being used and make sure that they all are accounted for before concluding the surgery. There isn鈥檛 even a need to manually count them, however: sponges can be outfitted with barcodes or tagged with radiofrequency tags 鈥 better known as RF tags 鈥 that send out a signal. A quick scan of a patient with a specially designed detecting wand alerts surgical staff to any sponges that are remaining inside the patient.

As it turns out, this technology would add about $10, at the most, to the cost of surgery. This is a drop in the bucket compared with the costs of additional treatment a patient needs if a sponge is left behind. However, only about 15 percent of hospitals currently use this type of technology. As a result, these mistakes continue to happen.
